Welcome to South Atlanta Urology & Gynecology Associates P.C.  Our physicians, along with our highly qualified staff, are pleased to provide patient care and surgical services to patients. Our doctors have privileges at Atlanta Medical Center, Emory Midtown University Hospital, Emory Johns Creek Hospital and Spalding Regional Medical Center.  Our offices are conveniently located in downtown Atlanta next to the Atlanta Medical Center. Also in Johns Creek, GA close to Emory Johns Creek Hospital along with satellite offices in Morrow, GA and Griffin, GA.


To schedule an appointment, you may call our main office number.  We provide same day care for emergency patients and most patients can be seen the same week they call. To expedite your appointment, please gather personal information about yourself and bring to the office:  your date of birth, social security number, address, health insurance ID cards, etc. We take your privacy seriously and have security measures in place to protect your personal health information.  If your regular doctor has referred you to our practice, we will coordinate care with your doctor’s office. Your doctor will receive feedback on your visit.

In order for our physicians to provide you with the best care, it is important that we gather information about your medical history, medications, and your current urologic and gynecologic problems. In addition, we bill insurance companies as a courtesy to our patients so current and complete health insurance information is necessary.

All new patients are required to complete our patient registration forms, for your convenience these can be found on our website under Patient forms, or you can receive a copy of these forms when you arrive at our office.    Please download and complete, and bring to your first visit.

We like to update your information periodically to make sure our records are current. Our nursing staff will update your medications and conditions with you at each visit before you see your doctor. Finally, to comply with federal rules designed to protect your identity, we ask patients to bring their health insurance card (if insured) and a photo ID to each visit.

Our doctors are participating providers for Medicare as well as many commercial insurance carriers. Because health insurance is a contract between you and a health insurance company, we advise our patients to check with their insurance company or employer to determine limitations of coverage prior to appointments. It is also very important that we have the correct insurance information or we won’t be able to bill the insurance company. Patients who do not wish to provide us with all the information we need to bill insurance will be treated as self-pay patients. As a convenience to you, we bill your insurance company for services rendered. Please remember, new identity theft protection laws require us to ask to see photo identification and health insurance cards. Insurance companies require us to collect co-payments at time of service, and we also collect deductibles and any shared patient responsibilities at time of service.

For patients with health insurance, we collect co-payments, deductibles and any shared patient responsibilities at the time of service and will submit an electronic claim to your insurance company. Patients receive detailed monthly statements and are required to make shared patient responsibility payments within 30 days of the receipt of their statement. Patients who are uninsured or self-pay are required to make full payment at time of service. We do offer prompt pay discounts for self-pay patients.
